Health Reimbursement Arrangement (HRA)

A Health Reimbursement Account allows you to use company provided funds to assist you in reaching your deductible. When you have a claim that is applied to your deductible, you will pay the claim from your HRA account first. Once all HRA funds are exhausted then you will be responsible for the balance. After you reach the deductible, you will move into a cost-share with the insurance company where you pay a percentage of the claims. This is referred to as co-insurance.

For Your Protection

The out-of-pocket maximum provides financial protection in the event of a serious illness or injury. The out-of-pocket maximum includes your payments for covered in-network or out-of-network expenses, as applicable, and is withdrawn from your funds. The out-of-pocket maximum, however, does not include penalties (such as a late cancellation fee for a doctor’s appointment). After you reach your out-of-pocket maximum, the plan covers all expenses up to 100% for the rest of the year.

Eligibility

You must be enrolled in the HRA-eligible medical plan.

Contributions

Blue Diamond Growers contributes the following towards your HRA:

  • Anthem PPO w/HRA: $1,000 Individual / $2,000 Family
  • Kaiser and Sutter Health Plus: $500 Individual / $1,000 Family

Kaiser and Sutter Health HRA will only cover the following services: Inpatient Hospitalization, Outpatient Surgery, Emergency Room, Skilled Nursing Care and Durable Medical Equipment.

Using Your Account

Eligible Expenses

You may use your HRA funds to cover eligible medical, prescription drug, and chiropractic expenses incurred by you and your eligible family members.

Your Debit Card

Use the debit card linked to your HRA to cover eligible expenses or submit a request for reimbursement of eligible expenses with a receipt.
